Program Disclaimer and Liability Waiver
Introduction
This disclaimer and liability waiver ("Agreement") is entered into by and between the Revive Recovery LLC, operated by Brant Cole ("Facilitator"), and the undersigned participant ("Participant"). By signing this document, Participant acknowledges and agrees to the terms and conditions outlined below as a precondition to participation in any program of Revive Recovery LLC.
1. Scope of Program and Nature of Services
Revive Recovery LLC is a faith-based recovery ministry aimed at helping individuals who are struggling with addiction to pornography and/or masturbation. Revive Recovery LLC is intended to offer guidance, support, and resources based on personal experience, pastoral counseling, and non-professional mentorship.
The Program is not a substitute for professional counseling, therapy, or medical advice. The Facilitator is not a licensed therapist, counselor, psychologist, psychiatrist, or mental health professional. The services offered are based on personal experience and non-professional training in counseling and related fields, and are delivered from a pastoral perspective. The programs of Revive Recovery LLC do not diagnose, treat, or provide medical or therapeutic advice for any mental health or medical condition.
2. Acknowledgment of Facilitator's Qualifications
Participant acknowledges that the Facilitator, Brant Cole, is a pastor with years of experience and training in addiction recovery, but is not licensed by any state or board as a therapist, counselor, or mental health professional. While the Facilitator may provide general counseling and spiritual guidance, such guidance is not intended to replace professional mental health or addiction recovery services.
3. Specific Outcomes or Results
Participant acknowledges that results from participation with the programs of Revive Recovery LLC may vary. While Revive Recovery LLC is intended to help individuals improve their quality of life and overcome pornography or masturbation addiction, there is not a guarantee of specific outcomes. Participant agrees to participate voluntarily and understands that their progress depends on various factors, including their commitment and effort.
4. Release of Liability
To the fullest extent permitted by law, Participant (on behalf of themselves, heirs, successors in interest, representatives, or assigns) releases, waives, and discharges the Facilitator, Revive Recovery LLC, and any associated entities, employees, or volunteers from any and all liability, claims, or demands arising out of or related to their participation in any of the programs of Revive Recovery LLC. This release includes, but is not limited to, claims for negligence, personal injury, emotional distress, expenses, costs, any damages of any kind at anytime, or any other harm that may arise during or after participation in any of the programs of Revive Recovery LLC.
Participant understands and agrees that they are solely responsible for their own decisions, actions, and well-being, and that the Facilitator is not liable for any adverse outcomes, mental or physical health issues, or other consequences resulting from participation in any of the programs of Revive Recovery LLC.
5. No Professional Advice
Participant acknowledges that any information, advice, or recommendations provided by the Facilitator during any of the programs of Revive Recovery LLC are based on the Facilitator's personal experience and pastoral training. The Facilitator does not provide professional, therapeutic, or medical advice, and any actions or inactions taken by Participant are their sole responsibility. Participant is encouraged to seek advice from a licensed healthcare or mental health professional if they feel they need or were recommended professional support.
6. Confidentiality
While Revive Recovery LLC will strive to maintain confidentiality, Participant understands that absolute confidentiality cannot be guaranteed in group settings. Participant agrees to respect the privacy of others in any of the programs of Revive Recovery LLC and maintain the confidentiality of shared information to the fullest extent possible.
7. Acceptance of Terms
By indicated their agreement below, the participant acknowledges that they have read, understood, and agree to the terms of this Agreement. Participants acknowledge that they have had the opportunity to ask questions about Revive Recovery LLC and its limitations, and that they are agreeing to this document voluntarily, with full knowledge and acceptance of its legal implications.
